The Agreement is generated electronically and is made available via MyFamiCord Panel; c) Agreement Conclusion Date – the date on which the Order is confirmed by the Service Provider; d) Biological Material – biological material stored by PBKM Sp. z o.o.; e) CryoSave AG – CryoSave AG with registered office at Chuerstrasse 65B, CH-8808 Pfaffikon, Switzerland; f) Customer – an individual (a parent) who had an agreement for storage Biological Material with – Previous Service Provider and who concluded the Agreement; g) FamiCord Group – a healthcare entity that renders a broadly understood medical, diagnostic, laboratory services related to and with a usage of the tissues and cells h) My FamiCord Panel – online panel where the Customer may access the Agreement, invoices and the storage certificate; i) Order Form – the electronic form which the Customer places to the Service Provider to order the Service. The Order Form is available at [https://www.famicordcryobank.ch](https://www.famicordcryobank.ch/) and in the My FamiCord Panel; j) PBKM – Polski Bank Komórek Macierzystych Sp. z o.o. with registered office at 29 Jana Pawła II Avenue, 00 – 867 Warsaw, Poland; k) Previous Service Provider – any company that has rendered services towards the Customer by collecting, processing, freezing and storing frozen biological material of the Customer prior to FamiCord Group. l) Price List – the list of prices for the Service and Additional Services provided by the FamiCord Group; m) Service – Biological Material storage by FamiCord Group; n) Terms and Conditions – these terms and conditions regulating the use of the Website and Service ordering; o) Website – the online portal owned by the Service Provider and available at [https://client.famicordcryobank.ch ](https://client.famicordcryobank.ch/)p) Working Day – any weekday from Monday to Friday that is not a public holiday in Switzerland, Poland, Portugal and Hungary. 3. Purchasing the Service via Website**** 3.1. In order to purchase the Service and the Additional Service, the Customer: a) Files in the Order Form; b) Accepts the Terms and Conditions, Agreement and other consents; c) Places the Order Form via the Website. 3.2. By placing the Order Form, the Customer confirms that: a) has stored Biological Material with Previous Service Provider; b) he/she is at least 18 years of age; c) have the legal capacity to sign the Agreement; d) the data submitted is complete and true; e) the Service Provider may use electronic means to exchange communication and conclude the Agreement. 4. My FamiCord Panel**** 4.1. My FamiCord Panel is created with a login (e-mail address indicated by the Customer) and password. In order to sign in, the Customer will have to enter an authorization code, received from the Website via e-mail provided in the registration process. 4.2. The Customer must secure the login and the password against unauthorized access. The Customer shall be fully responsible for any consequences of the My FamiCord Panel being accessed by third parties that come into possession of the Customer’s login and password due to Customer’s negligence. 4.3. My FamiCord Panel shall be disactivated after 30 days from the date of the last active Agreement termination. 5. Placing the Order and concluding the Agreement**** 5.1. Orders are received through the Website – 24 hours per day, all throughout the year, excluding any service interruptions caused by the Website maintenance or Internet service unavailability. 5.2. The Order shall be accepted by the Service Provider immediately following the Order being placed by the Customer. The content of the Agreement is available to the Customer in the process of placing the Order. 5.3. The Order will not be accepted if: 5.3.1. the Customer provided incorrect or false data when placing the Order; 5.3.2. the Customer cannot be contacted at the e-mail address or the telephone number provided in the Order Form. 5.4. Following the Order acceptance, the Agreement is concluded and the content of the Agreement is made available in My FamiCord Panel. 5.5. The Agreement is conditional upon the Service Provider conforming that the Biological Material is stored with PBKM together with necessary medical documentation. 5.6. The Customer will be informed immediately should the Service Provider confirmed that the Biological Material is not stored with PBKM. Upon receipt of such information by the Customer the Agreement will be cancelled. 6. Equipment requirements**** Requirements for the proper operation of the Website include a current Internet browser version, supported by the browser vendor, compatible with the HTML 4.0 standard or newer, accepting cookies and supporting JavaScript. 7. Customer’s Personal Data**** Information concerning the processing of personal data can be found in the Privacy Policy available on the Website [https://famicordcryobank.ch/privacy-policy](https://famicordcryobank-ch-joomla.empress.smarthost.pl/privacy-policy). 8. Additional Services**** 8.1. The Customer may choose that the Biological Material is stored by one of the FamiCord Group companies other than PBKM by indicating it in the Order Form. The Price List indicates the costs of the transportation from PBKM to the chosen location. Additional statements and consents might be required should the Biological Material is transported to a different location. 8.2. The Customer may choose other Additional Services indicated in the Order Form. Such Additional Services may be subject to additional costs provided for in the Price List and additional consents available in the Order Form. 9. Invoices and payments**** 9.1. The price for the Service is available in the Price List and will be finally calculated to the Customer before placing the Order and will be confirmed in the My FamiCord Panel. The prices presented constitute gross prices. 9.2. After the Agreement Conclusion Date the payment for the Service shall be payable within 14 days upon receipt of an invoice sent to the Customer (depending in the selected storage plan) in electronic form to the e-mail address provided by the Customer and through the My FamiCord Panel (the Customer shall also receive a link to the generated invoice via e-mail). Payments may be made using traditional wire transfer. 10. Termination of the Agreement**** The Customer has the right to terminate the Agreement for no cause within 14 calendar days from the Agreement Conclusion Date. A statement on termination shall be sent in writing by registered mail to the Service Provider or via email to [info@famicord.eu.](mailto:info@famicord.eu.) 11. Complaints**** Complaints regarding the Website, My FamiCord Panel and the Agreement shall be transmitted by e-mail at [info@famicord.eu](mailto:info@famicord.eu) or in writing by registered mail sent to the address of the Service Provider. 12. Final provisions** 12.1. These Terms and Conditions shall enter into force as of 14 October 2019. 12.2. The Service Provider reserves the right to amend these Terms and Conditions at its sole discretion. The amended Terms and Conditions will be published on the Website and available to the Customer in the My FamiCord Panel. Changing the Terms and Conditions shall not impact the rights and obligations of the Customer arising from the Agreement. 12.3. The Swiss laws shall apply to these Terms and Conditions and the Agreement and any other documents related thereto. 12.4. All disputes, claims, or controversies arising out of, or in relation to the Agreement or any other document related thereto, shall be settled in the appropriate Swiss common court with jurisdiction over such disputes, claims, or controversies. 12.5. Consent may be withdrawn at any time. If you withdraw the consent we can’t provide further commercial information about our products to you. Withdrawal of consent shall not affect the lawfulness of data processing performed until consent withdrawal. - **in order to fulfil legal obligations (Article 6(1)(c) of the GDPR) or perform tasks for the public interest (Article 6(1)(e) of the GDPR) and the equivalent provisions of the DPA.** As an umbilical cord blood bank and a healthcare entity, we are subject to a range of legal obligations concerning the storage of Biological Material; - **for the purposes resulting from legitimate interest pursued by FamiCord Group or a third party (Article 6 (1) (f) of the GDPR) and the equivalent provisions of the DPA.** If necessary, we process data in order to protect our own legitimate interests and those of third parties. Examples include ensuring IT security, enforcing and defending against claims, and ensuring biological material safety. **Art. 2 Categories of Data Processed** - The Personal Data you have provided to FCS is necessary for the purposes mentioned above and will include common and sensitive personal details that reveal your ethnic group, health condition and genetic data, and any information required by the Data Controller to achieve the purposes mentioned above. **Art. 3 Personal Data Recipient** - “Recipient” refers to the natural or legal person, public authority, service or other body to which the Personal Data is communicated, including third parties. A list of the names of these Recipients may be requested from FCS at any time. - As for the activity carried out by FCS, the Recipients may include: - companies connected, controlled, affiliated or otherwise involved with FCS, in particular companies belonging to FamiCord Group; - employees and collaborators involved in the data processing or in the provision of the services requested; - service companies, data processing companies, accounting/tax consultants and, in general, all bodies in charge of controlling and ensuring compliance with the purposes mentioned above; - the laboratories indicated in the Agreement or any other health entities working with the Data Controller, for cases pertaining to the performance of the contract, where the Data Controller guarantees that those entities will comply with the security measures to protect the data entrusted to them; - companies and/or commercial partners in general, such as manufacturers, communication companies, web service providers, partner websites and other professionals of the sector; - any third parties with whom we have stipulated specific agreements regarding measures to adopt in order to guarantee the security of the data entrusted to them. In any case data will be processed only to achieve the purposes mentioned above; - credit and insurance institutions that provide services functional to the purposes mentioned above. **Art. 4 Disclosure of Personal Data to Third Countries or International Organizations** - FCS has its seat in a third country within the meaning of the GDPR regulations. However, pursuant to Article 1 of the Commission Decision of 26 July 2000 (2000/518/EC), Switzerland is considered as a country providing an adequate level of protection for personal data transferred from the European Union. - The Data Controller shall not disclose Personal Data and/or data concerning Biological Material to third countries other than Switzerland or the United Kingdom or international organizations. If necessary, Data Controller has concluded suitable guarantees for the transfer within the meaning of the GDPR and the DAP. For the purpose of providing the service you requested, the data collected by FCS will be communicated and transferred to the laboratory indicated in the Agreement, which in any case acts in the name and on the account of FCS and guarantees the quality and security standards indicated by FCS. - The data is stored at BusinessPark Zug, Sumpfstrasse 26, 6302 Zug, Switzerland **Art. 5 Data Retention Period or Criteria to Establish It** - FCS will retain the data for the time required for the processing and established in the Agreement, in any case until the end of the relationship and after the end of such a relationship for a maximum period of 25 years. - After this time, data will be used only in anonymous format for statistical and analytical purposes. **Art. 6 Your Rights:** - As a data subject, you may send an e-mail to [dpo@famicord.ch](mailto:dpo@famicord.ch) at any time asking to exercise the following rights: - to access your personal data being processed, in accordance with article 15 GDPR and the equivalent provisions of the DPA, to verify whether or not your personal data is being processed and if it is, to verify the purposes of the processing, the categories of personal data processed, the categories of data processors, the period of retention of the data; to verify the ability to exercise your right to rectify, delete or limit the processing of your personal data; to verify the ability to complain to an appropriate data protection authority; to verify whether or not automated decision-making is applied and if it is, the logic used to apply it; - to rectify any incorrect personal data subject to processing, in accordance with Article 16 of the GDPR and the equivalent provisions of the DPA; - to obtain the deletion of your personal data subject to processing, in accordance with Article 17 of GDPR and the equivalent provisions of the DPA, without prejudice to any legal obligation to the contrary or the subsequent necessity to process the data for ascertaining purposes, or to exercise the right of defense in court, in the event the data is no longer needed for its original purpose; you revoke the consent to processing your data; the data has been processed unlawfully; the data must be deleted to comply with a legal obligation; - to limit the processing of your personal data, in accordance with Article 18 of GDPR and the equivalent provisions of the DPA, if the accuracy of the data is contested, only for as long as it takes FCS to verify its accuracy; the data has been processed unlawfully and you object to their erasure; supervened necessity to process the data for ascertaining purposes, or to exercise the right of defense in court; - to obtain a copy of your data in a structured, commonly used, machine-readable format and the ability to transfer your personal data from one controller to another, in accordance with Article 20 of GDPR and the equivalent provisions of the DPA, with regard to the personal data used in communications via e-mail containing news about FCS activities and the events organised by the company. - to withdraw your declaration of consent under data protection law at any time. Recently the results of similar procedures from an American institute (Duke University) were published; over 300 children were treated there. The decision to transfer the material was made by CryoSave AG registered in Switzerland, which is currently in the process of liquidation and does not provide the services to which it has committed in contracts concluded with clients. We are not a Party of contracts between you and CryoSave AG registered in Switzerland in liquidation, therefore we do not know what obligations resulting from the contract (if they were obliged to inform all Clients) are not able to provide the reason why the previous cryo preservation provider did not contact  all the Clients. It is worth mentioning that this was an emergency transport. The purpose was to prevent the sudden thawing of the material and was to ensure the continuity of the storage of the biological material FamiCord Group managed to secure samples of Clients of CryoSave AG registered in Switzerland in liquidation , but had no impact on its communication policy. We as FamiCord Group believe that the right to information is one of the key rights of the Client., so we try to reach out to all customers and inform them of the situation. - Will you confirm that material is stored in FamiCord Group? How quickly will such confirmation be sent? In order to check whether your material is stored with us, we kindly ask you to provide information that will enable us to identify the material sample and, at the same time, verify your legal right to this biological material. Therefore clients need to take a decision about further storage of them sample as soon as possible.****** FamiCord Group is fully aware of how important biological material containing stem cells may be in the future. Until a new agreement is concluded or a decision is made on further handling of the samples (e.g. their destruction), there will be no contractual regulations concerning remuneration for storage of the samples. In such a situation, FamiCord Group will be obliged to comply with the request of the entitled person but it may require the client to reimburse the market costs of releasing samples and their securing and storing in FamiCord Group’s laboratory. It must be also emphasized that in any case and in any time, FamiCord Group will comply with the applicable regulations of law and decisions of the official authorities concerning the storage of the biological material transferred to FamiCord Group laboratories. Your Child is the rightful owner of the stored materials, but since most Children are still minors, their parents or legal guardians are responsible for deciding on the future of these materials. Contracts create legal relations only between their parties, so execution of contract with FamiCord Group formally, does not affect existence of the contract with other entities and your rights arising from such a contract. We ensure that during the transfers, which were supervised by a group of experts, all the standards and requirements (technical, medical and legal) for the safe transportation of biological material were observed. Cryogenic containers were equipped with special sensors to measure temperature. Any opening of container for checking the samples was filmed. Additionally, all transfers were covered by FamiCord‘s Group insurance policy provided by a group of insurance companies. Whole transfer process was planned very precisely and with details with logistic specialists. Humancell ceased its cell banking activities at the end of October 2024, and its operating license was revoked. Before this happened, the Humnacell ensured the professional transfer and continued storage of the samples. The further storage of samples now continues at Europe’s largest stem cell bank, the FamiCord Group, in its ultra-modern, state-of-the-art laboratory in Warsaw. - Why isn’t the continued storage happening in Hungary? Because there is no cell bank licensed provider in Hungary that could immediately accommodate all the samples together. The FamiCord Group is the largest stem cell bank in and therefore has the necessary licenses and capacity to ensure the long-term, secure storage of the samples. - When did the transfer of samples occur? The transfer of samples took place in the second half of October 2024 - Who was responsible for the samples during the transfer? Was it still Humancell? The transfer was carried out at the request and under the order of Humancell. The transport company was the necessary licensed to transport the biological samples and appropriate insurance for the process. The transport was approved by the competent Hungarian authority - Who can I contact with my questions? Customer service is provided by the KRIO Intézet. We are happy to assist you. - What happens if I need the sample? The sample will be provided at the new storage location. If you need the stored biological sample for therapeutic use, please contact customer service and we will inform you about the process. - Is it possible for me, as a customer, to receive the sample in person? This is not possible because the samples are stored professionally in -190°C nitrogen vapor. Only a service provider with the appropriate technology and license can guarantee such storage conditions. Regulations stipulate that the sample can only be legally transferred to another cell bank or therapy center under close cooperation between the transferring and receiving service providers. Physical handover to private individuals is not permitted. - What happens to my Humancell client agreement? Does the contract have to be cancelled? No, the agreement does not need to be terminated because Humancell’s license has been revokes and your customer contract has been terminated. However, the FamiCord Group offers the opportunity to enter into a new customer contract, which will formalize the legal status of the stored biological sample. - Is it only possible to enter into a new storage agreement with a company belonging to the FamiCord Group? No. You have the right to control your sample. This includes transferring the samples to another cell bank at your own expense, requesting the sample to be destroyed, or entering into a new contract with the FamiCord Group at no additional cost for the pre-agreed storage period. If you choose to sign a new contract with the FamiCord Group, your pre-paid storage period with Humancell will be honoured, i.e. you will not be charged by the new provider until the pre-paid storage period has expired. Please formalise your new contract immediately to clarify the legal status of the samples. - Why must I decide on the frequency of payments for storage fees beyond the prepaid period at the time of contract signing? The contract between the parents and the FamiCord Group is a long-term, indefinite contract. After the prepaid storage period ends, the FamiCord Group recommends choosing either an annual payment model or prepayment for 5, 10, or 20 years. These payment obligations will only come into effect after the prepaid storage period ends. At the time of signing, no payment is expected from the parents. Importantly, during the contract term, clients retain the freedom to make decisions regarding their samples (including transferring them to another bank or requesting their destruction). In such cases, no storage fees will be due to the FamiCord Group. - What about the payments I have already made to Humancell? I prepaid for 20 years of storage. Is this lost? FamiCord Group honours the prepaid storage period with Humancell. Therefore, you have no payment obligations for the duration of the prepaid period specified in your Humancell contract. If you sign a new contract with the FamiCord Group, you can be assured that the legal status of your sample will be settled with the new provider without incurring additional costs for the prepaid period. For example, if you signed a 20-year contract with Humancell in 2007, your samples will be stored free of charge until 2027. - I was on an annual storage fee plan. Who do I pay now? If you were on an annual storage fee plan, you can sign a new contract with the FamiCord Group under a similar payment structure. In this case, the FamiCord Group will invoice you for further annual storage fees. We didn’t receive an invoice for the 2024 storage fee. Should this be paid retroactively to the FamiCord Group? Without access to Humancell’s payment records or invoices, we assume that clients have fully settled their contractual obligations. Clients who did not pay their service fees during the contract term owe the previous provider. The FamiCord Group assumes all payments documented in Humancell client contracts as fully settled and has no right to collect outstanding balances without a legal basis. - Can I transfer my sample to another cell bank? Yes, the child is the owner of the sample and the parents have joint control of the sample until the child reaches adulthood. This control allows you to transfer the sample to another bank at your own expense. If you find a willing bank, please contact Customer Services and the two banks will work together to arrange the transfer. - In which countries are FamiCord Group laboratories located? Can I choose where the samples are stored in the contract? The FamiCord Group has 11 laboratories storing samples in 8 countries, including Poland, Switzerland, Germany, Portugal, Hungary and other countries. The customer can choose between 3 storage location when signing the contract. The fee for the transfer of the biological sample from the FamiCord Group laboratory in Warsaw to another laboratory indicated in the contract is included in the price table of the contract. That said, it is recommended that the sample should be left where it is currently stored – in Warsaw, Poland. The FamiCord Group Central Laboratory in Warsaw, Poland, is the largest ultra-modern laboratory in the Group and complies with all legal and safety standards. - Do I have the possibility to have the sample delivered to the KRIO Intézet in Hungary? Yes, but in this case you will be charged for the transfer of the biological sample from the FamiCord Group laboratory in Warsaw to another laboratory specified in the contract, the amount of which is indicated in the price table of the contract. For the sake of the sample’s safety, we recommend that you avoid the additional risk of transport. The laboratory in Warsaw is the most modern stem cell laboratory in Europe. There is no additional benefit for you to return the samples to Hungary, as we can organise and manage the return shipment within the group if you need the sample for therapeutic purposes. - I don’t like the idea of having to contract a foreign company. In the past, I contracted a Hungarian company specifically so that I would not have to bear these legal risks. The following discount can be applied to the storage fee: for the second child – 20% discount on the list price - for the third child – 30% discount on the list price - for the fourth child – 40% discount on the list price The discounts are added based on the previous contract number in the online contracting interface. - How do I know for sure that you have my sample? After a successful delivery, the Warsaw laboratory staff will start the inventory, i.e. the individual checking of the samples received. This is expected to take about a year. We will then be able to confirm that your sample has indeed been transferred and is stored in the correct, undamaged condition. - Without my consent, by what right could the samples have been transferred? Given that Humancell has surrendered its operating licence and is no longer carrying out cell banking activities, it had to ensure that the samples were properly deposited and further stored in another laboratory with a cell banking licence. The termination of the activity provided the legal basis for the samples to be deposited in another cell bank. The process and the transfer were kept in constant consultation with the competent authority, which approved the transfer of the samples, thus contributing to their early safe storage. Of course, you have the right to control your samples, but in this situation, the priority was to ensure that they were safe as soon as possible. You now also have the possibility to exercise your right of disposal, which may include transferring the samples to another cell bank, requesting the destruction of the sample or transferring it to the FamiCord Group at cost until the end of the pre-contracted storage period. We can assure you that all standards and requirements (technical, medical and legal) for the safe transport of biological samples have been respected during the transport supervised by the expert team. The cryogenic vessels were equipped with special sensors so that the temperature was constantly monitored throughout the transport. The entire transport process was planned in advance with logistic experts in great detail and with great precision. No adverse events were detected during the transport. Since the transfer, the biological samples have been stored under strictly controlled conditions in FamiCord’s laboratory. Following the transfer of the samples, we have started a professional, individual inventory of the samples. Considering that there are about 15 000 samples, this process will be rather slow, taking into account the technological conditions (storage in nitrogen at -190°C) and is expected to take until the end of 2025. You can of course request the release of the sample for therapeutic purposes during this period. - What is the guarantee that the biological sample is our child’s? Together with the samples, the FamiCord Group has received a database containing the information necessary for identification. This information includes contract and sample numbers. Contract numbers and sample numbers are unique identifiers, assigned to one contract and one sample only. We believe that this data is fully sufficient to confirm that the biological sample belongs to your child. Furthermore, there have been cases where biological samples delivered to our laboratory have had to be released for medical treatment, the samples are in fact the property of the patients and there have been no complaints about their quality. - Is the FamiCord Group responsible for the quality of the samples stored? The FamiCord Group is only responsible for the biological sample from the moment it is received from your previous provider. For obvious reasons, the FamiCord Group cannot be held responsible for the quality, processing and storage of the sample prior to receipt; however, there is no reason to believe that Humancell has improperly processed or stored the biological sample, as its laboratory has all the necessary accreditation and has met the highest standards for the storage of biological samples. - What happens if the sample has to be used for transplantation? Are there any additional costs? The FamiCord Group has the most experience in Europe in the transfer of stem cells to transplant centres worldwide. This means that all standards (technical, medical and legal) for biological sample transfer are met. FamiCord Group supports the parents during the procedure and takes responsibility for the release of the samples to the clinic. During this process, we ask the parents to send us most of the available signed documents, such as the authorization to perform the tests on the reference sample and the exact date of delivery to the transplant centre. Once the mandatory tests have been completed, our Medical Director will authorise the release of the sample and the FamiCord Group will arrange for the safe transport of the samples to the clinic. If the parents have signed a new agreement with FamiCord Group and have included the transplant assistance package in their contract, we will not charge them any fees. The only cost on the parent/clinic side is the cost of the transplant patient’s genetic compatibility test (HLA test). If the sample is requested for autologous donation in Hungary, regardless of the final storage location indicated by the Parents in the Contract, FamiCord Group will provide the sample free of charge and transport it to the authorised hospital. Free delivery to Hungary applies only to standard therapies, not to experimental therapies. If the parents have not previously signed any contract with FamiCord Group, we need to sign a separate agreement for the release of the biological sample only, so that we have a legal basis to follow the release procedures. Parents will be charged a fee (at market rates) for the time it takes to sign the contract, for the testing and release of the sample, and for transport to the transplant clinic. Of course, once a special release contract is signed, parents can no longer purchase the transplant assistance package, so all logistical costs are then borne by the client. The legislation does not provide for an explicit procedure for handling “abandoned” biological samples. Rules allowing for the destruction of such samples, for example, are included in the contracts between the cell bank and the client. However, the FamiCord Group is not entitled to invoke the provisions of such agreements when deciding on the further handling of samples. The FamiCord Group is fully aware of the potential future importance of biological samples containing stem cells. Until a new agreement is concluded or a decision is taken on the further handling (e.g. destruction) of the samples, there will be no contractual arrangements for the remuneration of the storage of the samples. In this case, the FamiCord Group will be obliged to comply with the right holder’s request, but may claim reimbursement from the client for the market costs of releasing the samples and securing and storing them in the FamiCord Group’s laboratory. Overall we have checked the biological material located in 58 storage containers (out of which 55 were provided by former service provider). It is important to mention that the biological material collected from one child could have been divided into up to 8 different cryopreserved parts (subunits). Each of those subunits could be of different type of package, size and located in different storage containers. Each sample had to be scanned and matched with the customers’ records in our database. All of that made our work extremely difficult and time consuming. It was the largest project of its kind in the world. After checking over 800.000 subunits, we can confirm with certain reservation the overall acceptable condition of the inventory. We have found however some issues with less than 0,3% of subunits. Most of the issues which have been identified are the following: traces of the biological material on outers surface of the package, signs of mechanical damage of the package, lack of testing aliquots with the main sample. All of these issues are pretty typical results of not fully proper quality control and/or cryopreservation methods used by former service provider. The whole inventory was accumulated for almost 20 years of activity of former service provider and during that period different methodologies of freezing, coding and labelling were used by the company. The quality of electronic records and samples-related information provided by former service provider was very poor. Nevertheless we managed to correct most of these errors manually. We have found over 150.000 data (coding) errors which we thoroughly checked. The errors we identified include among others: the code of the subunit in the data was different from the code scanned from the subunit, the wrong location of subunits, additional numbers/signs or missing numbers/signs in the code, typos. Those electronic errors, however, had not affected the storage conditions of the samples but the may have hindered finding and use of the samples when needed In order to perform the process of verification timely and accurately we have created dedicated teams led by lab professionals. In total over 100 people were involved not mentioning dedicated call center and IT staff.
